If you have a medical reason not to fast, you shouldn’t. For example if you need to take medicine or are ill, you should skip that day and make it up after eid. If it is a problem that makes you not be able to fast for a long time, like my mother cant fast due to vitamin imbalances in her blood, (she needs to keep a constant diet). Than don’t fast. Your health is the most important thing you have and Islam doesn’t try to restrict that. Hope I could help :)
